Policy and Dental Insurance Information
Thank you for choosing
Columbus Dental Care. We are committed to providing you with the best
dental care regardless of your insurance coverage. While responsible for
the entire treatment, we ask you only pay your estimated patient portion
at the time services are rendered/completed until we know the final
figure paid by your insurance carrier. When the final insurance payment
is received, we will reconcile your account and we will bill or refund
you any difference. Please keep in mind that we deal with insurance estimates
only. Patients (Subscribers) can usually access a benefit booklet
from the Human Resources Department or from your insurance carrier
directly. In order to achieve these goals, we need to ask for your help
and understanding. You must realize, however, that:
1.
Your insurance plan is
a contract between you, your employer, and the insurance company itself.
Insurance co-pays, estimates, deductibles, and/or uninsured portions are
due in full at each visit. There are hundreds, if not thousands of
dental plans sold to employers, and we have no way of knowing all the
options or exclusions of each individual patient plan. We cannot be
responsible for any discrepancies with your plan.
2.
Our fees are
generally considered and fall within the "acceptable range" by
most companies, and therefore are covered up to the maximum allowance
determined by each carrier. This applies only to companies who pay on a
percentage (such as 50-80%) of "UCR" (Usual & Customary),
defined as fees usual and customary for this area. However, some
companies pay on a "fee schedule" - which means they will pay
a flat amount or % of their allowable fee. Other insurance companies
will reimburse the subscriber/patient directly. Not all services are a
covered benefit and many insurance carriers will arbitrarily select
certain services they will NOT cover. An insurance denial may be
contributed to a frequency limit, table allowance and/or coordination of
benefits clause. Refer to benefit booklet.
3.
Columbus Dental Care is
a participating provider with one dental plan, Northeast Delta Dental.
We are familiar and accept most other insurance companies, but are
considered out-of-network or non-participating providers with those
companies. This may reflect your coverage or insurance allowance. Refer
to benefit booklet.
4.
Columbus Dental Care is an "Amalgam Free" practice.
Amalgam (silver)
restorations have been used by dentists for decades with much success,
however, with today's newer materials and esthetic advances, the use of
amalgams has become obsolete. It is our belief that restoring all teeth
with composite (white/tooth-colored) restorations is providing the best
state-of-the-art care possible. You should be aware that your insurance
carrier may or may not cover composites at the same level as amalgam
restoration. Please contact your insurance company directly with any
questions.
We
emphasize again that as dental care providers, our relationship is with
you as our patient, and not with your insurance company or
employer. We will file your insurance claims as a courtesy, however all
charges become your responsibility from date of service and claims
unpaid by insurance after 60 days will become patient balance.
